how to get started
After we receive your referral for home health services, our home health agency will schedule an appointment with you. We will come to your home, review your healthcare needs, review your doctor’s orders, and then provide the collaborative care you require.
Referrals
Once you are referred to Granite Peak Home Health, we will respond in a timely manner and start treatment as soon as possible. Please fax your referral to: (406) 602-2088. Include your MD order, the face sheet, and your doctors referral. Thank you!
Our team will schedule a consultation.
We will come to your home and review all documents, doctors’ orders.
We will consult with your primary care provider.
In consultation with your primary care provider, we will provide the services you deserve.
